Changing your contact details
Update your name, address, phone numbers or email address.
Self-referral
You can self refer to Physiotherapy by collecting a form from the surgery and handing it back to your nearest Health Centre.
Self Refer to Podiatry by calling 0800 59 20 87 for any feet issues.
Self Refer to Maternity services by calling 0141 347 8422.
Refer yourself to a health service such as talking therapies without having to see a GP first.
Non-NHS Services
We are able to offer a range of services not covered under NHS treatment. This includes:
- Completing insurance forms
- Medicals for work, driving and insurance companies
- Reports for lawyers and DSS
- Completing holiday cancellation forms
- Vaccinations History
Many of these services incur a fee - details of these are available at reception. The practice has 28 days to complete non NHS work.
Please note that these forms will not be completed during a routine consultation with the doctor.
